Creating a Form

You can add any of the following fields in your form, and make any of them required or optional:

• Provide Email
• Provide Location
• Provide Phone Number
• Connect Platform: Instagram, Twitter, Discord, Telegram, TikTok, Unstoppable, Facebook
• Have a Stripe subscription
• Wallet: Connect, have a minimum balance, or hold a POAP
• Open-ended questions
• Multiple Choice: Choose between single select or multi-select options. Use “other” and add a field so users can create their own option.
• You can also add reCaptcha and Terms of Service to forms

Success settings

After successfully submitting the form, you can either send them directly into the membership, or show them a success screen. You can even include a custom call-to-action and automate an email based on successful submission.

Publishing the Form

Once your form is configured, publish it to make it live for users.

Integrating with Quests

On the Quests Tab, add a Kazm Form quest and select the form in the dropdown menu. This allows users to earn points by completing the form.
